In my experience the two most likely places that tights are likely to ladder are the toe and the heel.
You can do a lot to minimise the risk of this by keeping toenails short and smooth and your toes and heels soft and free from rough skin. Then there is the problem that men's shoes are not usually made with hosiery in mind. Women's still are, and are therefore smooth on the inside.
This is OK if you wear socks over your tights, but for those who want the sheer to ankle look, there is a real risk of roughness on a man's shoe causing the tights to ladder.
The answer to this may be to invest in a pair of sheer foot socks, sometimes called 'footsies'. These are usually invisible with shoes, because they are wholly inside the shoe.
They usually come in two-pair packs and you can get them in Marks and Spencers, Bhs and Boots, as well as other places. I keep some in beige and some in black and wear the nearest colour to the colour of my tights. These are 40 denier in 100% nylon from M&S, £3.00 for two pairs. They come in two sizes, S/M and M/L.
They really reduce the occurrence of ladders. These actually fit me, though my size 11 feet are probably at the extreme limit of their stretch. I find that slipping off the heel is not uncommon. This does not matter too much, because I find that the toe area is much more vulnerable than the heel.
